چکیده

A large literature documents the multiple roles clinical pharmacists can play in a variety of health care settings. Much of this literature focuses on measures of impact not directly relevant to this Report – eg, economic benefits, patient compliance, and drug monitoring. More recently, systems-based analyses of medication errors and adverse drug events (ADEs) have drawn attention to the impact clinical pharmacists can exert on the quality and safety of medication use. In this chapter, we review the evidence supporting the premise that direct participation of pharmacists’ in clinical care reduces medication errors and ADEs in hospitalized and ambulatory patients.

Clinical Pharmacist’s Role in Preventing Adverse Drug Events: Brief Update Review

Introduction In our original report, “Making Health Care Safer” 2001, Kaushal and Bates noted that over 770,000 people were harmed or died in hospitals annually from adverse drug events (ADE), with incidence rates in hospital-based studies ranging from 2 to 7 per 100 admissions. In the outpatient setting, as they also noted, one study on adults estimated the ADE incidence rate at 3 percent. The...

The role of pharmacists in clinical care: where do we go from here?

Americans fill over 3 billion prescriptions annually, and there are more than 10,000 prescription products available today (compared with 650 in the 1960s).1 Patients with chronic disease are often on more complex regimens, which can result in lower medication adherence and increased morbidity, adverse drug events, hospital admission, death, and health care costs.2–4 The importance of drugrelat...

Hemorrhagic events in patients receiving heparin or enoxaparin in combination with Oral antithrombotics, NSAIDs, and antiplatelet medications: A multi-center case-series study

Background: Hemorrhagic events are amongst the major adverse effects of injectable anticoagulants such as heparin and enoxaparin. This necessitates close monitoring of their use for clinically relevant drug-drug interactions. Aim: To study the rate of hemorrhagic events, the anatomical site, and the severity of the hemorrhage in patients receiving heparin or enoxaparin in combination with clop...

Choosing appropriate theories for understanding hospital reporting of adverse drug events, a theoretical domains framework approach

Adverse drug events (ADEs) may cause serious injuries including death. Spontaneous reporting of ADEs plays a great role in detection and prevention of them, however, underreporting always exists. Although several interventions have been utilized to solve this problem, they are mainly based on experience and the rationale for choosing them has no theoretical base. The vast variety of behavioral ...
